1. Return method: Submit the application form in person / by post.

2. Together with the documents required

• Completed and pasted photo application form
• 4 Self- addressed and stamped $2.2 envelope
• Apply for a copy of the child’s birth certificate
• The applicant must be at least one year old at the date of submission of application form
  

Remarks:

• Distribution and return of application form is available throughout the year
• OfficeTime: 8:00a.m. to 6:00p.m.(Mondays to Fridays) / 8:00a.m. to 1:00p.m.(Saturdays)
• Application Fee: No application fee is charged
